Entry Criteria
Are there any entry criteria to access the service?
The individual must have a dual diagnosis. Individuals are referred by a General Practitioner to the Mental Health Services. Following a Mental Health Assessment a Careplan is devised which identifies an individual needs. Referral is made by the Mental Health Multidisciplinary Team (MDT) or any team member.
